Privacy Policy

This notice explains what information this website collects, how it is used, how long it is kept, and the choices you have. It covers this website and our mobile app. Information you give us as a patient is handled separately (see “If you are a patient” below).

This website is run by Underground Strength and Wellness LLC (“Underground Strength & Wellness,” “we,” “our,” or “us”), which operates clinic locations in Georgia. For questions about this notice or your information, email selena@undergroundstrength.com or call (678) 865-8399.

The website has two kinds of forms:

• The contact form on our Contact page asks for your name, phone number, email address and a message.
• The Hormone Imbalance Quiz asks ten yes-or-no questions about symptoms, including sleep, mood (anxiety, irritability or depression), energy, strength and endurance, joint or muscle pain, sweating, headaches, overall well-being, and sexual desire or performance. It also asks for your full name and email address.

Quiz answers are information about your health. The quiz is a symptom checklist, not a diagnosis. Please do not put other medical details in a contact form message; call our office instead.

• A contact form message is emailed to our office. You may also receive an automatic email confirming that we received it.
• When you finish the quiz, your answers, name and email address are emailed to our office, and your answers are emailed to you at the address you entered.
• The website does not keep your submission. Contact form messages are not saved in the website’s database, and quiz answers are deleted from it automatically as soon as the emails have been sent.
• Emails from the website are sent through our email delivery provider, SMTP.com. For each email, the website keeps a short delivery record (who it was sent to and from, the subject line, the time, and whether it was delivered) and deletes that record automatically within about two days. The delivery record does not include the message itself. The subject line of a quiz email to our office includes your name.
• The copies that reach our office email are used to respond to you and follow up. You can ask us to delete them (see “Your choices and requests” below).

• Like most websites, the servers of our website host record technical information about each visit, such as your IP address, browser type and the page requested.
• Our security plugin, Wordfence, checks requests to the website for attacks. It records the IP address, browser details and page requested for requests it flags (such as blocked requests) and for sign-in attempts. Records of flagged requests are kept for up to 30 days; sign-in records are kept longer. Wordfence may share the IP addresses of suspected attackers with its security network.
• Our contact form counts how its fields are used (for example, whether a field was clicked or left empty) without recording what you type.

When you first visit, a banner asks whether you allow cookies for analytics and for measuring our advertising. Nothing in those two categories runs until you choose “Accept all” or turn a category on under “Manage preferences.” If you choose “Reject all,” they stay off.

• Necessary (always on): cookies the website needs to work and stay secure, including one named “ugs_consent” that remembers your cookie choice for six months. Your choice is stored only in your browser; we do not keep a copy of it on our servers.
• Analytics: Google Analytics, which shows us which pages are visited and how visitors find the website. If you allow it, Google Analytics sets cookies and receives your IP address, browser and device details, and the pages you view. Google keeps this data for the period set in our Google Analytics account.
• Advertising measurement: the Google Ads tag, which measures whether visits from our Google ads lead to someone contacting us. If you allow it, Google receives the same kind of information about your visit. We have turned off ad personalization and Google signals in the tags this website loads, so your visit is not used to add you to advertising audiences (“remarketing”) or to personalize ads. The website also tells Google’s tags not to send Google your personal details for advertising.

You can change your choice at any time with the “Cookie preferences” button at the bottom left of every page, or open your cookie preferences here. If you withdraw permission, the website stops loading those tags and deletes the Google cookies it can remove from your browser. You can also block or delete cookies in your browser settings.

Some pages load content from the companies below. The cookie banner does not control this content. When your browser loads it, the company receives your IP address and basic browser information, and some of these companies (such as Google, for maps) may set their own cookies.

• Google Fonts, for some text styles.
• Google Maps, for the maps on our Braselton, Johns Creek, Watkinsville and Woodstock pages.
• OpenStreetMap, for the map on our Clinic Locations page.
• Unsplash, for some images on our service pages.
• Elementor, for our accessibility menu.

We do not sell or rent your personal information. We share it only with service providers that help us run the website and deliver its email (our website host, SMTP.com and our security provider, Wordfence), with Google if you allow analytics or advertising measurement as described above, with the companies listed under “Other services our pages use” when your browser loads their content, when the law requires it, or when needed to protect patients or others from harm.

• Form submissions: not kept on the website (see “How our forms work”).
• Email delivery records: deleted automatically within about two days.
• Website backups: the website makes a backup copy of its database once a day and keeps the two most recent copies, so each copy is deleted after about two days. Backups can include the email delivery records and security records described above.
• Security records: flagged requests up to 30 days; sign-in records longer.
• Your cookie choice: six months, in your browser.
• Google Analytics and Google Ads data: kept by Google under the settings of our Google accounts.
• Emails in our office inboxes: kept while we respond to you and follow up. Ask us if you want them deleted.

You can ask what information we have about you from this website, or ask us to delete it, by emailing selena@undergroundstrength.com with the subject line “Data Deletion Request” or by calling (678) 865-8399. Please include your name and the email address or phone number you used, so we can find your information. We will confirm your request, respond within 30 days, and let you know when deletion is done. If we must keep something by law, such as a patient’s medical record, we will tell you.

Information you give us as a patient (at our clinics, by phone, during telemedicine visits, or through our OptiMantra patient portal) becomes part of your medical record. It is handled under our clinical privacy practices, separately from this website, and is kept for the time required for medical records. Ask our office if you have questions about your records.

If you are a patient, we may contact you by phone, email or text message about your care, for example lab draw reminders, lab review notifications, follow-up visits and treatment information, using the contact details you gave us. Call our office to change how we contact you.

We do not take payments through this website.

Our mobile app does not create user accounts and does not store your personal information on our servers. If you turn on push notifications, a device token is held by our notification provider only while notifications are on, and it is removed when you turn notifications off or uninstall the app. You can also ask us to remove your notification token using the contact details above.

Finding your nearest clinic. The app has a “Find My Nearest Clinic” button. When, and only when, you tap it, your device asks whether you want to share your approximate location. If you agree, the app compares that location with the addresses of our four Georgia clinics on your own device so it can show you the closest one first. Your location is never sent to us, never sent to anyone else, and never stored — not on our servers and not in the app. We never see it. If you decline, or if your device cannot determine a location, the app simply shows the full list of clinics and everything else works normally. You can change this permission at any time in your device’s settings. If you then choose to open directions in Apple Maps, Google Maps or Waze, the app passes only the clinic’s street address to that app — your own location is never included.

This website is not directed to children under 13, and we do not knowingly collect personal information from children through it.

Every page and form on this website uses an encrypted (HTTPS) connection. We use reasonable safeguards to protect the website, but no website or email system is completely secure. Please do not send information through the website that you would not want sent by email.

Our website links to other websites, such as our Facebook and Instagram pages. Those websites have their own privacy practices.

We may update this notice when the website or our practices change. We will post the new version on this page and change the “Last updated” date.
